By accessing our website, scheduling appointments, or using any of our health testing services, you ("you" or "user") agree to be bound by these Terms of Use and our Privacy Policy.

What We Are

QUANTUM Diagnostics is a health testing and specimen collection facility that provides:

Diagnostic testing (drug screening, sexual health testing, etc.)
Physical examinations (sports physicals, fit-for-duty exams)
Vaccinations and immunizations
Basic health screenings and laboratory services
Prescription services when medically necessary

What We Are NOT

QUANTUM Diagnostics is NOT:

A full-service medical clinic
An emergency medical facility
A substitute for your primary care physician
A hospital or urgent care center
Equipped to handle medical emergencies

3. Medical Disclaimer

No Doctor-Patient Relationship

Use of our website or scheduling system does not create a doctor-patient relationship. A professional relationship is only established when you receive in-person services at our facility and are evaluated by our licensed medical staff.

Not Medical Advice

Information provided on our website, in communications, or during consultations is for informational purposes only and should not be considered medical advice. Always consult with a qualified healthcare provider for medical concerns.

Limitations of Testing

Testing services are limited to the specific tests requested and performed
A negative test result does not guarantee the absence of disease or infection
Test results may require confirmation through additional testing
Some conditions may not be detectable during certain time periods (window periods)
We are not responsible for results interpretation beyond what is provided in your test report

Follow-Up Care

You are responsible for:

Following up on abnormal or positive test results with a healthcare provider
Seeking appropriate medical care based on your results
Informing sexual partners of positive STI results as required by law
Taking prescribed medications as directed

4. Licensing and Certifications

Professional Staff

All QUANTUM Diagnostics staff members are appropriately licensed and certified, including:

Certified Specimen Collectors
DOT-Certified Collectors (for Department of Transportation testing)
Licensed Nurse Practitioner(s) operating under a Collaborative Practice Agreement with a licensed physician
CLIA-certified for rapid screening tests

Laboratory Partners

Laboratory testing is processed through:

LabCorp
Quest Diagnostics
Other CLIA-certified laboratories

We are a collection facility. Results generated by external laboratories are provided as reported by those laboratories. QUANTUM Diagnostics is not responsible for laboratory errors or delays beyond our control.

Prescribing Authority

When medically necessary, prescriptions may be written by our licensed Nurse Practitioner operating under a Collaborative Practice Agreement with a supervising physician in accordance with South Carolina law.

5. Services, Pricing, and Payment Terms

Pricing

All prices are listed on our website and are subject to change without notice
Prices listed are for self-pay patients only
We do NOT accept insurance

Payment Policy

Payment is required at the time of service
We accept cash, credit cards, and debit cards
All sales are final

No Refunds

ALL SERVICES ARE NON-REFUNDABLE. Once a test is performed, a specimen is collected, or a service is rendered, no refunds will be issued under any circumstances, including but not limited to:

Change of mind
Duplicate testing
Dissatisfaction with results
Test results that do not meet your expectations
Failure to appear for the result consultation

6. Appointment and Scheduling Policy

Scheduling

Appointments may be scheduled online through our website or by phone
Walk-ins are accepted based on availability
You must create an online account to schedule appointments and receive results

Cancellations

You may cancel or reschedule appointments by contacting us at least 24 hours in advance
Late cancellations (less than 24 hours) will be noted in your account

No-Show Policy

This policy is strictly enforced with no exceptions:

A "no-show" occurs when you fail to appear for a scheduled appointment without prior cancellation
After TWO (2) no-shows, you will be permanently banned from receiving services at any QUANTUM Diagnostics location for ONE (1) YEAR from the date of the second no-show
This ban is automatic and applies to all QUANTUM Diagnostics facilities
No exceptions will be made for any reason

7. Age and Consent Requirements

Minors

Patients under the age of 16 must be accompanied by a parent or legal guardian who must provide written consent for services
Patients age 16 and older may consent to services on their own behalf

Parental Access to Records

Parents and legal guardians of patients under 16 have the right to access their child's medical records in accordance with applicable law. For patients 16 and older, access to medical records is governed by state and federal privacy laws.

8. Test Results and Turnaround Times

Result Delivery

Test results are provided through your secure online account
You will receive a notification when results are available
Results are NEVER shared via unencrypted email or text message

Turnaround Times (ESTIMATES ONLY)

Rapid/Instant Tests: Results provided same day (typically within 15-30 minutes)
Laboratory Tests: Results available within 7-10 business days (estimates only)

IMPORTANT: Turnaround times are estimates only and may be delayed due to factors outside our control, including laboratory backlogs, holidays, specimen quality issues, or shipping delays. We are not liable for delays in result delivery.

Abnormal or Positive Results

We will make reasonable efforts to contact you regarding abnormal or positive results
You are responsible for checking your account regularly for results
Failure to receive or review results does not relieve you of responsibility for follow-up care

9. Limitation of Liability

To the Maximum Extent Permitted by Law:

QUANTUM DIAGNOSTICS, QUANTUM STAFFING AND RESOURCES LLC, AND ITS OWNERS, OFFICERS, EMPLOYEES, AND AGENTS SHALL NOT BE LIABLE FOR:

Indirect, incidental, consequential, or punitive damages
Lost profits, lost data, or business interruption
Damages resulting from delays in testing or result delivery
Laboratory errors by third-party testing facilities
Adverse reactions to vaccines or medications
Complications arising from medical procedures
Failure to diagnose or detect medical conditions not within the scope of requested testing
Reliance on information provided on our website

OUR TOTAL LIABILITY TO YOU FOR ANY CLAIM SHALL NOT EXCEED THE AMOUNT YOU PAID FOR THE SPECIFIC SERVICE GIVING RISE TO THE CLAIM.

Insurance

QUANTUM Diagnostics maintains professional liability insurance as required by law.

10. Assumption of Risk

By using our services, you acknowledge and accept the following risks:

No medical test is 100% accurate
False positive and false negative results can occur
Testing may not detect all conditions or infections
You may experience adverse reactions to vaccines, medications, or procedures
There are inherent risks in any medical procedure or treatment

You voluntarily assume all risks associated with our services.
